We respect the confidentiality of people who contact Cardiocare and limit the information requested through public website forms.
How website information is handled
Cardiocare uses information submitted through approved contact routes to respond to enquiries, coordinate appointments and support requested services. Public website forms, community discussions and the patient portal should never be used for emergencies.
Patient portal records
Home vital-sign entries are stored under the email address of the signed-in account so that only that account can request its saved timeline. The portal stores measurements and optional notes provided by the user. These entries are not monitored live by Cardiocare staff and do not automatically become a clinical instruction or emergency alert.
Heart Failure Community membership
Membership uses the signed-in account, a community display name, a Nigerian mobile number, the member’s relationship to the group and recorded consent. The phone number is used for SMS verification and is not shown to other members. Accounts remain limited until a Cardiocare administrator approves access.
Forum, chat and member stories
Approved members can post in the private forum and group chat. Cardiocare administrators may review, hide or remove content to protect the community. Member stories are published only after the contributor gives consent and a Cardiocare administrator approves them. Do not post medical records, passwords, account numbers, contact details or another person’s private information.
Medical information
Educational pages, calculators, charts, support-group discussions and symptom-routing tools are general information. They are not a diagnosis, treatment plan or substitute for individual clinical assessment. A clinician’s personal targets and advice take priority.
Contact details and record requests
Only provide information needed for the requested communication. Clinical records and sensitive documents should be shared through an approved secure hospital pathway. Contact Cardiocare if you need help reviewing access to information associated with your account.
